The following forms may be printed out from your own computer if you have the Adobe Acrobat Reader installed. Click on the Adobe graphic to the right to download and install it on your PC if necessary.

Here are the forms you need for admission to the undergraduate programs at Shasta Bible College:

• Application for Admission (Choose one...)
• .pdf form to download and fill out by hand
• MS Word form to fill out on computer before printing out
• One-page Essay Form
• Foundational Documents
• Reference Forms (Be sure to print 4 of the two-page documents)
• Transcript Request Form (Be sure to print one form for each school from which you need a transcript)
• I-20 Information Request Form
